Tumescent Liposuction history
Liposuction is also referred to as lipoplasty, suction-assisted lipectomy and liposculpture. It is the most popular cosmetic surgery today in fat loss. Since its inception in 1974, great methodological and technological advances have created more options for patients. One of the most utilized techniques in liposuction is the tumescent technique.

The procedure is relatively new to the cosmetic surgery scene, having come into widespread use in the 1990’s.It was first used by the dermatologist Dr. Jeffry Klein.

In the past, liposuction required patients to have pre-operative blood transfusions, since the loss of blood was so significant. The tumescent liposuction technique is relatively recent and is considered one of the safest options in liposuction; this technique can greatly reduce the swelling, pain, and bruising usually associated with the surgery. Though not completely risk-free, the tumescent liposuction method is a good option for many patients.

Generally the fat is removed from thighs, chin, buttocks, love handles (abdomen), breasts and stomach for women and chin, waist, breasts and buttocks for men.

Tumescent liposuction is done using a local aesthetic. A large volume of saline solution containing the aesthetic and a drug called "Epinephrine" is injected directly into the areas where there are excessive fatty deposits. Epinephrine is important in the surgical process because it shrinks capillaries and minimizes blood loss.

At the start of the Tumescent liposuction procedure, the surgeon anesthetizes the skin at select locations using tiny needles. Once the skin has been numbed, longer needles are used to anesthetize the subcutaneous fat with large quantities of the dilute lidocaine and epinephrine solution. Patients are frequently given a mild sedative, such as Valium to help relax them during the procedure. Once the areas to be treated are completely numb, the doctor begins the Tumescent liposuction procedure. Small holes are made in the skin using round skin biopsy punches to form access sites for the liposuction microcannulas. A microcannula is a very small stainless steel tube, ranging from 1mm to 3mm in diameter, designed to effectively remove small amounts of fat. Fat is suctioned out of the body in small tunnels through the microcannula, leaving the fibrous strands attached to the skin and muscles intact. Once the desired fat has been suctioned out, the microcannulas are removed. Since the adits are such small holes, there is no need for sutures using this method. This actually serves to speed up the healing process since drainage can occur via the adits, greatly reducing the amount of swelling and bruising of the treated areas.

Furthermore the local anesthesia that is contained in the solutions remains in the treated area for approximately 24 hours benefiting the patient in minimizing post-procedure pain.

Rhytidectomy, also known as a facelift, is a plastic surgery procedure in which all the excess fat between the nose and mouth is removed, and the underlying muscles are tightened. What to expect during and after a face lift procedure

Do not worry too much about scarring, because the incision will be made on the hairline, where it is less noticeable. It is advisable to spend at least one day in the hospital after a face-lift.

Cosmetic Surgery Face Lift (Rhytidectomy) Procedures


Traditional face lift - A typical face lift, performed under anaesthetic, involves the surgeon making a surgical cut right around the hairline, past the front of the ears. Cuts may also be made under the chin, to lift a sagging jaw line. The skin will then be separated from the underlying muscles of the face. Excess fat will be removed and the muscles will be tightened. The skin will then be lifted up, pulled back and any excess will be trimmed off. It will then be stitched back to the line where the cut was initially made.

Another option is a face lift using keyhole (endoscopic) surgery, mainly for a brow lift (where the forehead is lifted). Several tiny cuts are made that allow an endoscope to be placed under the skin. The operating instruments are inserted through other tiny holes. The surgeon watches a TV monitor to see what he is doing under the skin.

Another type of face lift surgery is minimal access cranial suspension (MACS). Shorter cuts, compared to a traditional face lift, are made in the temple and in front of the ears. Fibrous tissues under the skin are tightened with permanent stitches that are fixed to tissue close to the cheek bone or the fibrous connective tissue under the skin.

Both endoscopic facelift and MACS have the advantages of smaller scars and usually a shorter recovery time.

Fat deposits - 12-year-old gets plastic surgery

When it comes to having plastic surgery, how young is too young?

Some say it depends on the circumstance.

At just 12-years-old, Brooke Bates, Pflugerville, says liposuction was her only hope in battling a persistent weight problem. She says it's completely changed her life.

Brooke Bates now 13 has struggled with her weight for most of her life.

"All my friends weighed 70 or 80 pounds and I was weighing 218," said Bates.

It wasn't until last year that she found a solution.

"I knew it was going to be good, but I didn't know it was going to be this good...I used to be a size 18, 20, 22, and now I'm a size 8‑10," added Bates.

At 12‑years‑old, Brooke had liposuction and soon after, a tummy tuck, but she says getting the surgery wasn't something she and her family took lightly.

Brooke says her weight was affecting her health both physical and emotional.

"It hurts when kids look at you different and when kids talk about you, and then the little whispers and stuff, it hurts," said Bates.

"We've spent a lot of time dieting, exercising, and a lot of tears and it was all over in just a few short months," added Cindy Bates, Brooke's mother.

Plastic surgeon, Doctor Robert Ersek, who performed Brooke's surgery, says initially, he believed Brooke was too young and denied the request from her father, Joey Bates — who was recently diagnosed with cancer.

After several months of counseling, Dr. Ersek changed his mind.

"One of the things that tipped it over for me was the fact that she said that she wanted her dad to see her in a dress before he died," said Ersek.

Dr. Ersek ultimately removed more than 40 pounds of fat and skin.

"I would not do liposuction and abdominalplasty on a 12‑year‑old girl or boy," said Plastic surgeon, Dr. David Turner.

He believes children that young rarely realize the risks, and they're still growing.

"Children come into puberty; proceed through adolescence ages 12 to 14 or so. Most children have a growth spurt. They frequently grow taller, their limbs, their arms and legs get longer, a lot of physical changes happen and at 12 years old, those changes probably have not had a chance to develop completely," added Ersek.

Brooke has come a long way since then.

"I look at the pictures from 6 months ago, and say, no, not ever going back there again. Never," said Brooke.

She says her new figure has given her the motivation she needs to keep the weight off. She's now lost a total of about 60 pounds.

Her father, Joey Bates, is in remission.

Photocure has finalized its first Phase II acne study. In this dose selection study, 43 patients were treated with three different doses of MAL cream and four different durations prior to light exposure.

The objective of the study was to select the best dosing regime for the Phase II confirmatory dose response study which is planned to start later this year.

The proposed acne treatment involves an application of Photocure’s patented MAL cream, followed by illumination with the company’s Aktilite lamp.

Results showed that the best dosage to reduce the number of inflammatory lesions is with lower cream concentrations and shorter light exposure sessions.

The study was conducted at two Canadian centers.

A fast-growing franchisor of laser and skin care clinics is speeding up its growth by taking its products to the Internet and its franchises to Asia.

Dermacare LLC is preparing to launch Web sales within two months of its own skin care line, developed over the years at its first clinic in northeast Phoenix. It will offer five to six products for a variety of skin types, ranging from cleansers and moisturizers made with green tea extracts to sunscreens and serums to hydrate and smooth the skin.

"I put together a team of medical professionals, nurses and aestheticians to critique and analyze what we wanted in the skin care products, what was necessary, what was snake oil, what wasn't necessary," Dermacare founder and CEO Carl Mudd said. "We put together a very solid value-priced line for consumers."

The Phoenix-based franchisor also is negotiating to award its first international franchise in Pakistan within two months. Other agreements are in the works for China, India, Malaysia and Taiwan, Mudd said.

The company already has 26 clinics open in the U.S., with 18 more due to open by December. An additional 140 are under contract; Dermacare's goal is to have 500 U.S. clinics within three years.

Consumers' concern with their appearance, coupled with improved technology, are driving the growth of the skin-care clinic business, Mudd says.

"For a lot of people, beauty is not a luxury item," he said. "It's a staple. It's a requirement."

Medical spas were the fourth-largest but the fastest-growing segment of the $11.2 billion spa industry from 2002 to 2004, according to the most recent statistics from the International Spa Association. The number of medical-spa locations grew 109 percent during that period, compared with 26 percent growth in locations for the industry as a whole.

Mudd, who spent eight years as a franchisor in the real estate business and four years as a software distributor, says his move into the skin care business wasn't that much of a stretch. He got interested in the industry when a friend asked him to invest in a laser hair removal clinic.

But at that time, it was hard for clinics to sort out equipment manufacturers' claims and even harder for consumers to sort out the safety and effectiveness of procedures, Mudd said. There was no dominant company offering a range of procedures, and dermatologists or plastic surgeons may not know the latest technologies, he said.

"By taking all of the mainstream, highly effective laser and aesthetic procedures and putting them under one roof under a great trade name and driving it with great marketing, and by distinguishing ourselves by focusing on safety and medical professionals, I started out," he said of the company's launch in 2001.

The Dermacare name, however, didn't come to him initially. But the second time he made a list of 200 possible names, it leapt off the page. It was such a natural description of what the company does that even when he started the first clinic people insisted they had heard of the company.

Mudd started Dermacare's first clinic at Tatum and Shea boulevards, intending from the beginning to perfect the system for franchising.

He interviewed hundreds of doctors, nurses and patients to narrow the technology to three to four laser systems that could perform multiple functions in the clinics. He created management systems to keep franchisee operations on track. And he spent more than $1 million testing coupons, TV ads and everything in between to come up with a multimedia marketing plan to reach Dermacare's target market: middle-class women ages 25 to 50.

"We didn't have a scantily clad 19-year-old in the marketing piece," he said. "We had . . . an attractive 40-year-old woman who could have been any woman."

The customer base has since expanded to include more teens, minorities and, as ego barriers come down, men. The most popular procedures are laser vein removal, Foto Facial light treatments, Botox and dermal filler treatments.

The prototype clinic took three years to perfect, and Dermacare sold its first franchise in 2004. Most of the franchisees are physicians, ranging from dermatologists who want to expand their business to obstetricians driven from their specialty by high insurance costs.

Mudd says Dermacare doesn't want to compete with the local hair salon or high-end boutiques. He doesn't even like being called a medical spa, contending that medical procedures and spa procedures don't mix.
